A case manager keeps track of the moving parts of your care so you don't have to hold it all in your head.
In a typical session, you and your case manager sit down and sort through what needs attention:
- Housing, insurance paperwork, and getting to appointments
- Coordinating therapy, medication, and any outside providers
- Setting small next steps and checking on the last ones
Our licensed clinicians fold this work into your wider treatment plan. As you make progress, priorities shift, and we adjust the plan with you so the support fits where you actually are.
